Model Application Form

BOOKING CONDITIONS

General

Model rates are based an individual basis. Rates are determined on models resume, experience and titles. Please contact our office for rates.

Travel

Models will travel to various locations, all expenses paid. This includes airfare, hotel, travel expenses to and from location plus $35 per diem per model, for food expenses. Half day rates for travel days may be factored into models booking rate depending on destination location.

Hair/Make up/Prep Time

Will be discussed prior.

Half & Full Day Shifts

Day rate bookings are based on an 8 hour shift. Half day rate bookings are based on a 4 hour shift. There is a minimum of 4 hours for all bookings.
Service charge - An agency service charge of 20% will be factored in on all bookings and usage fees.

Cancellations

Must notify AC PRODUCTIONS of cancellations within 7 working days, or a fee of 10% of the total amount on invoice will be charged.

Video & Film

Bookings involving motion picture, film or video recording must be negotiated in advance.

Lingerie/Underwear

To be done on a closed set and the client must specify type of lingerie.

Changing Facilities

Client must provide private changing facilities on all bookings including location.

Booking Form

This form to your right must be filled out completely and submitted to us to complete our booking process. Form will be forward on to models being hired.
